OPENING

1. Promoter

The promoter of this competition is Form Dental Ltd, 65 London Road, Stapleford, Cambridge, CB22 5DG, United Kingdom. This promotion is not sponsored, endorsed, administered by, or associated with Instagram (@formdentaluk), Facebook (@formdentaluk), or TikTok (@formdental).

2. Prize

One winner will receive a Smile Makeover Package consisting of:

• • Invisalign treatment (value £3,500+)
• • Professional whitening (value £595)
• • Total prize value: £4,095+

Five runners-up will each receive:

• • Professional teeth whitening treatment (value £595 each)

The prize includes:

• • Consultation and digital scan
• • Invisalign treatment where clinically appropriate
• • Whitening treatment

The prize is non-transferable, non-exchangeable, and no cash alternative will be offered.

3. Clinical Suitability

• • All dental treatment is subject to clinical examination and suitability.
• • If the winner is not clinically suitable for Invisalign treatment, the Invisalign component of the prize will be replaced with Form Dental treatment credit of equivalent value.
• • If the winner is not clinically suitable for teeth whitening, the whitening component of the prize will similarly be replaced with Form Dental treatment credit of equivalent value.
• • Where the winner is not suitable for both treatments, the full prize value will be provided as Form Dental treatment credit.
• • Treatment credit may be used toward any dental treatments available at Form Dental, subject to clinical suitability.

4. Eligibility

Entrants must:

• • Be 18 years or older
• • Be a UK resident
• • Be able to attend Form Dental in Cambridge for treatment
• • Join the Form Dental waiting list via the official entry page

5. Entry

To enter the competition participants must:

• • Join the Form Dental waiting list
• • Follow Form Dental on Instagram (@formdentaluk), Facebook (@formdentaluk), or TikTok (@formdental)
• • Tag at least 3 friends in at least one of the competition posts on any of the three social media platforms and like the post

Only one entry per person is permitted.

6. Competition Period

The competition opens on the date of the post going live and closes at 11:59pm on 5th April 2026. Entries received after this time will not be included. All waitlist members who signed up prior to this competition launching are automatically entered and do not need to re-register.

7. Winner Selection

• • One winner and five runners-up will be selected at random from all valid waitlist entries, including those who joined before the competition launched.
• • The winner will be contacted via email and by phone (if a number was provided at the time of waitlist registration) within 7 days of the closing date.
• • If the winner does not respond within 7 days of being contacted, Form Dental reserves the right to select another winner.

8. Redemption

The winner must:

• • Attend an initial consultation within 6 months of being notified
• • Begin treatment within 12 months of the consultation

Failure to attend scheduled appointments may result in the prize being withdrawn.

9. Treatment Limitations

• • The prize covers the value stated only.
• • If the winner chooses treatments exceeding the prize value, they will be responsible for paying the difference.
• • Treatment plans are determined solely by the clinical judgment of the dentist.

10. Publicity

By entering the competition, entrants agree that Form Dental may use the winner's name, photographs, and treatment results for promotional and marketing purposes, including social media, website, and advertising, unless the winner requests otherwise.

11. General

Form Dental reserves the right to:

• • Amend or withdraw the competition if necessary
• • Substitute the prize with an alternative of equal value
• • Disqualify entries that do not comply with these terms
