Form Dental is a fully private, independent dental practice — founded by Dr Lara Phillips, who grew up in Cambridge and has lived in Great Shelford since childhood.
We are based at 65 London Road, Stapleford, with free on-site parking and easy access from Cambridge city centre, Trumpington, Great Shelford, and all surrounding villages.
Our ethos is simple: appointments that are never rushed, clinicians who listen, and a calm environment where every patient feels genuinely cared for. We are welcoming new patients.

What makes Form Dental different?
As a fully independent practice, we are not part of a corporate group or NHS contract. Every decision we make is focused solely on what is best for our patients — not on targets or throughput.
Do you see adults and children?
Yes. We welcome patients of all ages. Children under six are seen free of charge, and we are experienced in caring for anxious patients and those who have not visited a dentist for some time.
How far is Form Dental from Cambridge?
Stapleford is on the southern edge of Cambridge, around 10-15 minutes from the city centre depending on your starting point. We are particularly convenient for patients in Trumpington, Great Shelford, Sawston, and surrounding South Cambridgeshire villages.

Calm and relaxed atmosphere right from the door. Friendly receptionist and a warm welcome from Geeta the dentist. Put my mind at rest straightaway about procedures and practices at the surgery. Geeta explained everything in detail after an examination of my teeth that was probably the most explorative check I've ever had in all my years. No hard sell and a genuine authentic explanation of my teeth (and related oral health). High tech equipment and no uncomfortable processes to map teeth and gums. In my views this is a totally professional service that's almost worth looking forward to.
Form is everything that a dental practice should be! I was nervous before attending but Laura and Andrea were professional and kind and put me at ease. Highly recommended!
This is hands down the best dentist I have ever visited in my life. It's like being at a 5 star resort/spa. They take a great deal of care ensuring the patient understands the rationale behind the treatment plan and procedures.
