When can I join?
The Practice Plan will be available from our opening day on 20th April 2026. You can register your interest now by contacting us, and we will be in touch as soon as sign-ups open.
Is this dental insurance?
No. The Practice Plan is a membership scheme that covers your routine preventive appointments and provides a discount on treatment. It is not an insurance policy and does not cover the cost of additional treatments — though members receive 10% off any treatment they need.
Can I cancel?
The plan has a minimum term of 12 months. After that, you can cancel with 30 days' written notice. If you need to cancel within the first 12 months, we will review your account: if no included appointments have been taken, we will cancel immediately and refund all payments in full. If appointments have already been used, those will be charged at their standard rates and any difference settled at the point of cancellation.
Does the 10% discount apply to all treatments?
Yes. Members receive 10% off all additional treatments at Form Dental, including cosmetic treatments such as Invisalign®, veneers, composite bonding, and teeth whitening, as well as restorative treatments such as fillings, crowns, and implants.
Can I add family members?
The plan is open to adults aged 18 and over. Each plan is per person, and we are happy to set up individual plans for multiple family members. If you are a member, any children under the age of 6 in your household will still receive their complimentary check-up as normal — no separate plan is required for them.
Terms & Conditions
1. Minimum term
The plan has a minimum term of 12 months from the date the first direct debit is collected. After 12 months, the plan continues on a rolling monthly basis and can be cancelled with 30 days' written notice. Early cancellation within the first 12 months is subject to the conditions set out in clause 7.
2. Included appointments
The plan includes 2 dental check-ups and 2 Guided Biofilm Therapy (GBT) hygiene appointments per plan year. Appointments that are not used within the plan year do not carry over and cannot be redeemed in a subsequent year.
3. Treatment discount
Members receive 10% off additional treatments carried out at Form Dental while their plan is active. The discount cannot be used in conjunction with any other offer or promotion.
4. Non-transferable
The plan is personal to the named member and cannot be transferred, shared, or used by another person.
5. Missed payments
If a direct debit payment is missed or returned unpaid, plan benefits — including the treatment discount — will be suspended until the outstanding amount is settled. Form Dental reserves the right to cancel the plan if payment is not received within 30 days of the missed due date.
6. Price review
Form Dental reserves the right to review the monthly plan fee annually. Members will be given at least 60 days' written notice before any price change takes effect. Members who do not wish to continue at the revised price may cancel during the notice period without penalty.
7. Early cancellation
If you wish to cancel within the first 12 months, we will review your account. If no included appointments have been taken, the plan will be cancelled immediately and all payments refunded in full. If appointments have been used, those will be charged at their standard rates and any difference settled at the point of cancellation.
8. Practice's right to cancel
Form Dental reserves the right to terminate a membership with 30 days' written notice. In the event of termination by the practice, any unused portion of the current plan year will be refunded on a pro-rata basis.
9. Eligibility
The plan is available to individuals aged 18 and over and cannot be used for treatment at any other dental practice.
