Why join
Rather than paying for each appointment individually, your plan spreads the cost into a simple monthly direct debit. No surprises, no large one-off bills.
Both plans offer a saving compared to paying per appointment. Complete members save £30 a year on their routine appointments before any treatment discounts.
Every time you need additional treatment — from a filling to Invisalign® — you receive 10% off as a plan member. For patients who invest in their smiles, the savings quickly add up.
Regular check-ups and professional hygiene are the most effective way to prevent dental problems. The plan is designed to keep you on track with the appointments that matter most.

When can I join?
The Membership Plan is available now. You can sign up by getting in touch with the practice or asking the team at your next visit, and we will set up your direct debit.
Which plan should I choose?
The Essential plan at £18.50/month is ideal if you prefer one check-up and one hygiene visit per year. The Complete plan at £35/month gives you two of each — the frequency most dentists recommend for maintaining good oral health. If you are not sure, we are happy to advise at your first appointment.
Is this dental insurance?
No. The Membership Plan is a membership scheme that covers your routine preventive appointments and provides a discount on treatment. It is not an insurance policy and does not cover the cost of additional treatments — though members receive 10% off any treatment they need.
Can I cancel?
The plan has a minimum term of 12 months. After that, you can cancel with 30 days' written notice. If you need to cancel within the first 12 months, we will review your account: if no included appointments have been taken, we will cancel immediately and refund all payments in full. If appointments have already been used, those will be charged at their standard rates and any difference settled at the point of cancellation.
Does the 10% discount apply to all treatments?
Yes. Members receive 10% off all additional treatments at Form Dental, including cosmetic treatments such as Invisalign®, veneers, composite bonding, and teeth whitening, as well as restorative treatments such as fillings, crowns, and implants.
Can I add family members?
The plan is open to adults aged 18 and over. Each plan is per person, and we are happy to set up individual plans for multiple family members. If you are a member, any children under the age of 6 in your household will still receive their complimentary check-up as normal — no separate plan is required for them.
Can I upgrade from Essential to Complete?
Yes. If you join the Essential plan and later decide you would like to upgrade to Complete, simply get in touch and we will arrange the change from your next billing date.

Terms & Conditions
1. Minimum term
The plan has a minimum term of 12 months from the date the first direct debit is collected. After 12 months, the plan continues on a rolling monthly basis and can be cancelled with 30 days' written notice. Early cancellation within the first 12 months is subject to the conditions set out in clause 7.
2. Included appointments
The Essential plan includes 1 dental check-up and 1 Guided Biofilm Therapy (GBT) hygiene appointment per plan year. The Complete plan includes 2 dental check-ups and 2 GBT hygiene appointments per plan year. Appointments that are not used within the plan year do not carry over and cannot be redeemed in a subsequent year.
3. Treatment discount
Members receive 10% off additional treatments carried out at Form Dental while their plan is active. The discount cannot be used in conjunction with any other offer or promotion.
4. Non-transferable
The plan is personal to the named member and cannot be transferred, shared, or used by another person.
5. Missed payments
If a direct debit payment is missed or returned unpaid, plan benefits — including the treatment discount — will be suspended until the outstanding amount is settled. Form Dental reserves the right to cancel the plan if payment is not received within 30 days of the missed due date.
6. Price review
Form Dental reserves the right to review the monthly plan fee annually. Members will be given at least 60 days' written notice before any price change takes effect. Members who do not wish to continue at the revised price may cancel during the notice period without penalty.
7. Early cancellation
If you wish to cancel within the first 12 months, we will review your account. If no included appointments have been taken, the plan will be cancelled immediately and all payments refunded in full. If appointments have been used, those will be charged at their standard rates and any difference settled at the point of cancellation.
8. Plan upgrades
Members on the Essential plan may upgrade to the Complete plan at any time. The revised monthly fee will apply from the next billing date following the upgrade request.
9. Practice's right to cancel
Form Dental reserves the right to terminate a membership with 30 days' written notice. In the event of termination by the practice, any unused portion of the current plan year will be refunded on a pro-rata basis.
10. Eligibility
The plan is available to individuals aged 18 and over and cannot be used for treatment at any other dental practice.
