Come in for a check-up
New patients usually start with a comprehensive examination. Your dentist will assess your teeth, gums and overall oral health, take any necessary x-rays, and talk through anything you would like to improve.
Your personal plan
After your assessment we will explain our findings clearly and, if any treatment is needed, provide a written plan with transparent costs before anything goes ahead. No pressure, no surprises.

Form Dental is an independent private practice and we do not offer NHS dental care. Your new patient examination is £95, including any necessary x-rays. A hygiene appointment with our gold-standard Guided Biofilm Therapy is £130. You will always receive a clear written estimate before any treatment begins.

Membership
Spread the cost from £18.50 a month
Many of our new patients choose a membership plan after their first check-up. Your routine check-ups and hygiene visits are covered by a simple monthly payment, and you receive 10% off all additional treatments.

Calm and relaxed atmosphere right from the door. Friendly receptionist and a warm welcome from Geeta the dentist. Put my mind at rest straightaway about procedures and practices at the surgery. Geeta explained everything in detail after an examination of my teeth that was probably the most explorative check I've ever had in all my years. No hard sell and a genuine authentic explanation of my teeth (and related oral health). High tech equipment and no uncomfortable processes to map teeth and gums. In my views this is a totally professional service that's almost worth looking forward to.
